AI関連の仕事を求めて就活を始めて気づいたこと。ネイティブが受けるような英語による試験は歯が立たない。何を言っているのか掴むのが非常に難しい。Googleの言葉遣いが理解できる人にとっては、もしかしたら易しいかもしれない。
What I realized when I started job hunting for an AI-related job: Tests in English, like those taken by native speakers, are hard to solve. Very difficult to understand what they're saying. It may be easier for people who can understand Google's language at least.

日本語の試験を用意している会社は非常に少ない。日本語の試験にはめったにお目にかかれないと思われる。
There are very few companies that offer Japanese language exams. It seems that you rarely see Japanese language exams.

しかし、これらの会社はIndeedやランサーズなどに、日本語による求人情報を出している。試験も多分日本語によるものが多くなるはず。ただ、英語の試験も課される。
However, these companies post job information in Japanese on sites such as Indeed and Lancers. Most of the exams will probably be in Japanese as well. However, an English test will also be required.

AIの評価の仕事は2年間ほど続けてきたのに、フリーランサーであるためか継続性がなかった。親会社のプロジェクトが1つ終わると、別のプロジェクトを請け負った下請けに採用してもらうことになる。英語の試験にはなかなか受からない。

クリニックで6月の血液検査の結果について説明を受けた。数値はのきなみ改善した。血糖値は144、正常より高いがこれは食後2時間ほどでの検査だからそうなって当然。HbA1cは6.3。正常範囲から0.1だけ出た。血圧は171、高い。

I went to see the clinic doc to discuss the results of my June blood tests. My numbers have generally improved. My blood sugar level was 144—higher than normal, but that’s to be expected since the test was taken about two hours after a meal. My HbA1c was 6.3, just 0.1 outside the normal range. My blood pressure was 171, which is high.

あしゆびの潰瘍については縮小してきたとこちらから伝えたが、そのときにずっと疑問だった潰瘍表面の白いものの正体について尋ねたら、白癬菌ではなく白苔（はくたい）だとのこと。菌などから成るコケのことで、舌ゴケに似ている。白苔があると潰瘍の治りが遅くなるそうだ。

I told him that the ulcer on my little toe had started to shrink, but when I asked about the white substance on the surface of the ulcer—something that had been puzzling me for a while—he said it wasn’t a fungal infection but “hakutai” (white moss). これからはAIがますますヒトから職を奪うようになるから、ヒトは別途活路を見出す必要がある。肉体労働ができればそれがいちばん。しかし、それができないヒトは、投資に向かうのが最善かもしれない。不動産投資は危ない話が多すぎるから避けて、危なくない話が多い株に手を出すのが良いと思う。競馬と比べるのが適切かどうかは脇に置いておいて、競馬と比べると株ははるかに投資として安定している。

Since AI will increasingly take jobs away from people from now on, people need to find other ways to make a living. If you can do manual labor, that’s best. However, for those who can’t, turning to investing might be the best option. Since real estate investing involves too many risky ventures, it’s best to avoid it and instead turn to stocks, which generally offer fewer risks. Setting aside whether it’s appropriate to compare it to horse racing, stocks are far more stable as an investment than the racing.


私の場合、資金不足が最大のネックだが、ポイ活で証券会社に口座を作り、この約2、3年間でもと値の倍以上に利益を上げた。買ったのは米国株ばかり。日本は100株単位だから貧乏人には買いにくいが、米国は1株単位なので気軽に株主になれる。AI関連株の安いのを買っておいて2，3年放っておいたら結構な額上がってくれた。たとえ2，3万円しか買えないとしても、買うものを選べば見返りは大きい。

杖をついて歩くうちに、今日は速度があがったよう な感じがした。歩幅も大きく、足取りもしっかりしていた。近くの中央公園に出かけて景色を眺めてきた。

As I walked with my cane, I felt like I was moving a bit faster today. My strides were wider, and my steps were steadier. I went to the nearby Sayamadai Central Park and took in the scenery.

右足のABIについて
About the ABI of the Right Leg


右足首のABIは0.48だった。つまり悪い方の脚、左足よりも成績が悪かった。ずっと右は正常だが左は...という考え方をしていたのに、ここにきて数値が逆転した。

The ABI for my right ankle was 0.48. In other words, it was worse than the left one, which has the apparent problem. I had always thought that the right side was normal but the left wasn't... but now the numbers have reversed.


CTスキャンによる映像では、右脚には左脚のような詰まりは観察されなかった。太い血管が写っていた。しかし0.48しかないということは、膝下にある1箇所の詰まりが大きな影響を及ぼしているとみられる。

The CT scan image showed no blockage in the right leg similar to that in the left leg. A large blood vessel was visible. However, given that the value is only 0.48, it appears that a single blockage below the knee is having a significant impact.


今後は右も鍛えないといけないということになるが、この数値の割には右は良くて右脚のふくらはぎが痛くなって歩行を続けられないということは、これまで起きなかった。

This means I’ll have to start training my right side as well, but despite these numbers, my right side is actually in good shape—I’ve never had a situation before where my right calf started hurting so much that I couldn’t keep walking on.

病院での検査の日。ABI（ankle brachial pressure index;上肢下肢血圧比）を機械装置で測る。以前に別の病院で計ったときには左足は血圧０。数値が出なかった。クリニックの医師が触診で「脈が触れない」としたのと同じ結果だった。

It was the day of my hospital test. They measured my ABI (ankle-brachial index) using a machine. When I had it measured at a different hospital before, the blood pressure reading for my left ankle was 0—the machine didn’t register a value. This was the same result as when the doctor at the clinic said, “I can’t feel a pulse,” after performing a physical examination.


それが今日は0.54になった。0.0が0.5以上になった！
Today, however, it went up to 0.54. It went really from 0.0 to over 0.5!


これはお祭りだと思った。しかし、担当医師は少しも動じず、こちらの努力をほめもせず、0.5までの数値上昇は無視し、以前のCTスキャンの古い映像にこだわった。検査結果説明の日なのに（おまけに11日に受けた検査の結果説明は無視されたし）。

I thought this was some kind of farce. However, the doctor in charge didn’t bat an eye; he didn’t praise my efforts, ignored the 0.5-point increase in my numbers, and kept fixating on the old image from a previous CT scan. And this was supposed to be the day he explained my test results (not to mention that he had already ignored the results from the test I took on the 11th).

自然治癒を目指す本人がいる。本人は手術には否定的。しかし、一方、バイパス手術を勧める勢力がある。

手術は自然に人間の手を加えるということ。すると自然は変化し、歪んだ不自然に変わるリスクがある。今、あしゆびの潰瘍も治ってきたし、歩く速度も上がった。できれば自然はそのまま守るべきだろう？

ただ、やっぱり反対派とのえんぎりはできない。病状が急変したときに病院に受け入れられないと困る。だから保険。ここしばらく、完治（ABI=0.9以上）までは、これら勢力の存在にも我慢する必要がある。

However, I simply can’t cut ties with the opposition. If my condition suddenly worsens, I’d be in trouble if the hospital wouldn’t admit me. That’s why I need insurance. For the time being, until I’m fully recovered (ABI ≥ 0.9), I’ll have to put up with these groups.

Blockages in the coronary arteries are scary because they can lead directly to death. If I ever experienced chest pain so severe it felt like my chest was being squeezed, I’d want to be rushed to the emergency room and have surgery. In that situation, I wouldn’t be in a position to argue about the relationship between surgery and nature.


冠動脈の血管の太さは、下肢の血管とあまり変わりがないようだ。太くて5ミリ、末梢で1ミリ程度らしい。

The diameter of coronary arteries appears to be not much different from that of blood vessels in the lower limbs. The thickest are reportedly about 5 millimeters, while those in the extremities are about 1 millimeter.

6月9日の記事に書いた「脚の付け根の方の股のところにある「えら」みたいなものはたぶん硬化した血管」について、チャッピーに訊いた。どうも間違いらしい。太い動脈や血管はもっと骨に沿って存在するもののようだ。腱だろうとのこと。

I asked Chappy about what I wrote in my June 9 post—that “the ‘gill-like’ thing near the groin at the base of the leg is probably a hardened blood vessel.” Apparently, I was wrong. Large arteries and blood vessels run closer to the bone. He or she said it’s probably a tendon.

太ももの動脈の血管は1センチほどの太さがあるが、膝下には太い血管はなく、足先などでは2mmほどの細さになるらしい。これほど細いと頻繁に詰まって不思議はない。

The arteries in the thighs are about 1 centimeter in diameter, but there are no large blood vessels below the knee, and they apparently become as thin as about 2 millimeters in the toes and other parts of the foot. Given how thin they are, it’s no wonder they get blocked so often.

病院の予約で皮膚灌流試験というものを受験してきた。ASO用の検査で、皮膚の毛細血管の働き具合を調べる。毛細血管の働きが弱い場合、栄養が行き届かないから傷や潰瘍などがあっても治りが遅く、悪くなる。ASOの治療方針を立てるときには欠かせない検査のようだ。

I went to the hospital for an appointment to undergo a skin perfusion test. This test is used for ASO and examines the function of the skin’s capillaries. If capillary function is poor, nutrients cannot reach the affected area, causing wounds and ulcers to heal more slowly or to become worse. It seems to be an essential test when developing a treatment plan for ASO.


私の場合、自身の動脈の詰まり具合にはまったく自信がなかったが、毛細血管については自信があった。実際、ノートパソコンのバッテリーを足の甲に落としたときに負った比較的深い傷は、皮膚の蓋の仕方がうまくいったのか、2，3日で完治していた。また、ある方から壊疽と呼ばれた、左足第2趾の潰瘍も、爪の再生こそ遅かったが、すでに治っていた。だから体験に基づく自信があった。

In my case, I had absolutely no confidence in the condition of my arteries, but some about my capillaries. In fact, a relatively deep wound I sustained when I dropped my laptop battery on the top of my foot had healed completely in just two or three days—perhaps because the skin had closed over it so well. Also, the ulcer on the second toe of my left foot—which someone had called gangrene—had already healed, even though the nail was slow to regrow. So I had confidence based on my own experience.


今回の検査技師は看護師らしかったから結果を訊いてもむだで、医師からの説明は次の受診時になる。とても丁寧な看護師さんで、脚の向きを変えたり休憩を取ったりしてくれた。面白かったのは医療従事者でラップを使う人を見たのは、この人が初めてだった。巻いていったラップの替わりになる医療用ラップをチビっと分けてくれた。台所のサランラップとあまり変わらないものみたいだった。

The technician this time seemed to be a nurse, so asking him for the results would have been pointless; the doctor will explain them at my next appointment. He was a very kind nurse who adjusted the position of my leg and let me take breaks. What was interesting was that this was the first time I’d ever seen a healthcare professional use plastic wrap. He gave me a small piece of medical-grade plastic wrap to use instead of the regular kind I’d brought on my toes, which seemed pretty much the same as Saran Wrap for kitchen.

I think those "lump-like" things in the groin area near the base of the thigh are probably hardened blood vessels. 医院の待合室などに置かれている腕を入れて測る血圧計は、アームイン（スポットアーム）血圧計と呼ぶようだ。AIによると、「巻き方に個人差がないため誰でも簡単に正確な測定ができる」とのこと。しかし、測定のベースについて考えると、もともと個人個人にある個人差には考慮が払われていない。その点を考えに入れてから言えであって、そうじゃないと必ずしも正確とは言えないのじゃないだろうか。というのも、ふだん家庭でカフ式の血圧計で測る数値との乖離が激しすぎるからだ。

The blood pressure monitors found in waiting rooms at clinics and other facilities, where you insert your arm to take a reading, are apparently called "arm-in" (spot-arm) blood pressure monitors. According to AI, since there are no individual differences in how the cuff is wrapped, anyone can easily take an accurate reading. However, when considering the underlying principle, it does not account for the natural individual differences that exist among people. It should only be considered accurate after taking those differences into account; otherwise, it cannot necessarily be called accurate. This is because there is a significant discrepancy between the readings and those taken with standard cuff-style blood pressure monitors used at home.


私の場合、家で測るとたいてい148〜165くらいの範囲に収まる。しかし、クリニックでは180〜198にもなる。「白衣高血圧、病院で測ると高くなる高血圧の人はたしかにいます」と医師は言う。しかし、「それも治療の対象」なのだそうだ。なぜならば「治療しないとその種の場面からダメージを受けるから」というりくつらしい。

In my case, when I measure at home, it usually falls within the range of about 148 to 165. However, at the clinic, it can go up to 180 to 198. The doctor says that there are indeed people with white coat hypertension, whose blood pressure rises when measured at the hospital. However, that is also subject to treatment. The reasoning seems to be that if it is not treated, one can be damaged in such situations.


それもそうかなと思った。だから2剤併用も受け入れた。やってみようという気になった。

軟膏剤の塗り方については野口皮膚科医院のページを参照。私の場合、ラップはぐるぐる巻きにしていた。が、この医院はラップを適切な大きさにカットして患部に当てている。なるほどそのほうが賢い。

本日はクリニックの日。大雨の中を車で送ってもらった。クリニックの名称が変わっていた。先生が膝がひどく悪くなり、診療に穴を開けてしまったので院長を交代したそうだ。防衛医大当時の匍匐前進と関係があるのかどうか。そういえば2年前にお世話になった別の病院の防衛医大卒の女医さんも脚が悪かったなぁ。

Today was a clinic day. I was driven there in heavy rain. The clinic's name had changed. The director was replaced because the doctor's knee got so bad that it put a hole in the clinic. I wondered if it had anything to do with the creeping operation at the time of the National Defense Medical College. Come to think of it, a female doctor at another hospital who graduated from the same college and took care of me two years ago  also had a bad leg.

処方は高血圧、高血糖、潰瘍、水虫の各治療薬でいつのまにか9種類に増えた。すさまじい。

ところで傷などの治療に際して軟膏などの薬を塗ってラップでくるむのは、きちんとした皮膚科の医学用語でODT療法(Occlusive Dressing Technique)と言うそうだ。ラップなどで密封すると薬効が何倍にもなるという。私のやり方もそれだった。ただし、ラップを巻いたときに、密着のためかその部分が非常に痛くなり耐え難くなることもあって、他のドレッシング材のほうがいいとも思える。綿の靴下とか。

By the way, applying ointments and other medicines to wounds and wrapping them in plastic wrap is called ODT therapy (Occlusive Dressing Technique) in proper dermatological medical terminology. The effectiveness of the medicine is said to increase many times over when sealed with plastic wrap. My method was the same. However, when the wrap is wrapped, the area can become very painful and unbearable, probably due to the close fit, so other dressings may be better. Cotton socks, for example.

たしか5時半頃トイレに行った。その後、また寝て目覚めたらスマホが8時5分だと言う。寝坊した。顔を洗ったとき、寝ぼけ感があった。この感覚とは若いころぐっすり寝たあとに経験したとき以来の再会だった。起きてカツサンド1かけらとコーヒーの軽い朝食をとると、なんとすっきりした寝覚め感が出てきた。至るところのつっかい棒がなくなり、からだじゅうがすっきりしている。人間はこんなことで生き返るのか、なんだ、要するに寝坊すればいいんか。

I think I went to the bathroom around 5:30. Then I went back to sleep and woke up and my phone said it was 8:05. Overslept. Washing my face, I had a sleepwalking sensation. I hadn't experienced this since my youth after a good night's sleep. Woke up and had a light breakfast of a piece of pork cutlet sandwich and coffee, and to my surprise, I started to feel refreshed and awake. The pinches were all gone, and my whole body felt refreshed. I wondered if this is how people come back to life, or what, all I had to do was sleep in.

脚の付け根の方の股のところに「えら」みたいなものがあることに最近気づいた。エラ、顔の造作で「えらがはっている」と言われる、あのえらみたいな出っ張りが股の内側の左右両方にある。これが左右で大きさや硬さが違うようなので、たいへん気になっている。いったいこの部分はなんなんだろう。鼠径部の鼠径管は鼠径部の筋肉の中を通るトンネルのことだから別物。いったいなんなの？

I recently noticed that I have something that looks like a "gills" at the crotch at the base of my legs. The gills, or the protrusion like gills that is called "era" in Japanese in facial features, are located on both the left and right sides of the inner side of the crotch. I am very concerned about this because the size and hardness of these protrusions seem to be different on each side. What in the world is this part? The inguinal canal in the groin is a tunnel that passes through the muscles of the groin, so it's a different thing. What on earth is it?

推測があたっているかどうかわかりませんが、当サイトの訪問者のうち日本以外は、日系の人たちや海外在留の日本人の人たちじゃないかと思っております。米国にいる日系の人たちや在留邦人の人数はどのくらいになるかAIに訊いたところ165万人程度になるとのことでした。これはだいたい福岡市の人口と同じです。だから何だということじゃなく、ただただこの推測をもとにサイト運営にあたっております。読者が見えないままブロッグのサイトを運営するのもなかなか骨の折れるものなんですが、あまり気にしないでどーでもいいやというスタンスになってきております。

I am not sure if my guess is correct or not, but I think that the visitors to this site, except for those in Japan, are people of Japanese descent or Japanese living abroad. When I asked AI how many Japanese Americans and Japanese residents in the U.S. are, I was told that it would be about 1.65 million. This is roughly as large as the population of Fukuoka City. I am not saying that this is so, but I am just using this guess to manage the site. It is quite a challenge to manage a blog site without knowing who the readers are, but I have come to take the stance that I don't really care and that it doesn't matter.

これは画期的なことかもしれない。ラップを2重に巻いた。ただそれだけなのに、具合が大変良い。もちろん水虫の治療のことを言っている。とんでもなく痛むときはオロナインとみずむしクリームそれぞれ大量、それに滴下水虫薬、マキロン噴霧してアルコール除菌ティッシュで巻き、その上からラップを1重で巻いてきた。しかし、今日は除菌ティッシュは小指だけでラップを2重にしてみたところ、あたりもやわらかいし、最近流行の傷の湿潤療法にもなり、良さそうなのだ。もし同病の方がいたら、ぜひおためしを。ラップ1重のときに発生するラップが張りついて痛む感じがない！

This could be a breakthrough. I wrapped a double layer of plastic wrap. That's all I did, and I feel very well. Of course, I am talking about athlete's foot treatment. When I had a tremendous pain, I used to use a large amount of each of Oronine and Mizu-mushi cream, plus drops of athlete's foot medicine, a spray of Makiron, wrapped with alcohol sterilizing tissue, and then wrapped in a single layer of plastic wrap over that. Today, however, I tried wrapping the wounds with two layers of plastic wrap with only the fifth toe wrapped with sterilizing tissue, which seems to be a good idea because it is soft to the touch and can be used as a moist therapy for wounds, which has recently become a popular trend. If anyone else has the same problem, please give it a try. I don't have the feeling of the wrap sticking and hurting that occurs when using a single layer of wrap!

膝は肉質はほとんどなく、骨ばかりと言っていいくらいガチャガチャで、ロボットの部品みたいな構造をしている。曲げ伸ばししたり、ひざまづいたりするのに便利にできている。足は骨の多さに特徴がある、膝はそのしくみが足と同様あまりにも複雑でおそれいる。誰が作ったんだろ。

肉づきのあまりよくない装置だけれど、膝窩（しっか）動脈という動脈がまんまんなかを通っている。膝を折った状態にして、手指を3本くらいまとめて膝窩の窩（か＝くぼみ）の奥に押し込むと、どくどくしている拍動がわかる。どくどくしていないと、脚が下のほうで死んでいるかもしれない。

It is not a very fleshy device, but an artery called the popliteal artery runs right through the middle of it. If you fold your knee and press three fingers together deep into the hollow of the popliteal fossa, you can feel its throbbing pulse. If it is not throbbing, the leg may be dead below the knee.

脚の膝から下、また足について、血管がどうなっているか手探りでさぐっている。ヒザ下には主要な動脈は全部で3本あって、うち1つは内くるぶしのかかと寄りの内側にある。これはどうやら足底の土踏まずにつながっているらしく、土踏まずはこれまで冷たかったけれど、最近やや温かくなってきたように思える。

I am groping my way down my leg from the knee down and about my foot, trying to figure out where the blood vessels are. There are three major arteries in total, one of which is located on the inner side of the inner ankle near the heel. This artery seems to be connected to the sole of the foot, which used to be cold but seems to have become slightly warmer recently.


あとはスネの骨沿いの動脈が今日になってやっと開通したような気がする。足首のインステップから甲に、さらに足先まで至っているもの。これは膝の裏あたりに電動マッサージャーを当て、振動で刺激したら開通したようだ。スネの骨を中心にして死んだような肌触りだったのが、温かみが戻り、感触がしっとりしてなめらかになった。確かな効き目があった。

I also have an artery along the bone of the shin that seems to have finally opened up today to my joy. The one that leads from the instep of the ankle to the instep and then to the toes. This seems to have been opened by applying an electric massager to the back of the knee and stimulating it with vibration. The skin, which had been dead to the touch around the bones of the shin, returned to warmth and became moist and smooth to the touch. The tool had a definite effect.


あともう1本は、スネよりもっと外側に硬い長いものがあり、これが血管じゃないかと思えるのだが、これは足の外側に通じているように思える。この部分はまだ死んでいる。

There is another one, a long hard one, further out than the shin, which I suspect is my hardened blood vessel, but this one seems to lead to the outside of the foot. This part is still dead.


以上、間違っていたらだいたい100年後に訂正しようと思う。
If I am wrong about the above, I will correct it in about 100 years.
 

ASOについては、血管内科の医師が言った「からだを動かしてください」が大きなヒントになった。「歩いてください」ではなく、「動かして」である。歩くのはたしかにいいことだけど、それよりもできれば血管を動かして、できれば溜まったゴミを移動させるほうが内部がきれいになる。

As for ASO, a great clue came from a vascular physician, who said, "Move your body. Not "walk," but "move your body." Walking is good, yes, but it is better to move the blood vessels, if possible, in order to move the accumulated debris, if possible, to clean the inside of the vessels.

 足白癬について、間違った考え方をしていたことを正しておきたい。大きな勘違いというものは私にはつきものなのだ。

I would like to correct a wrong idea I had about tinea pedis. A big misunderstanding is something that comes with me.


1つは前にも言ったとおり、趾の指と指の間の赤い凹み はただの凹みではないということ。これは趾間型という水虫。

One is that, as I mentioned before, the red indentations between the toes are not just indentations. This is an athlete's foot called interdigital type.


最近気づいたもう一つの重大なことは、薬は足全体に塗るべきだということ。以前はかかとや、土踏まずよりもっと前の部分にばかり薬を塗っていた。そのような硬い角質の多い部分に菌が巣食っていると思っていた。ところがいっこうに痛みが収まらない。カロナールが必需品になっていた。しかし、ところが、先日寝る前に足全体に塗ったところ、実に平和な夜になった。何で？と思えるほど痛みが収まった。そうか、全体に塗ると全部死滅してくれる、そうしないと、残ったのが夜間に活動する、と気づいた。

Another significant thing I recently realized is that the medicine should be applied to the entire foot. I used to apply the medicine only to the heels and the areas further forward than the arch. I thought the fungus was nestled in those hard, calloused areas. However, the pain did not stop at all. Caronal had become a necessity. The other night, however, I applied it to my entire foot before going to bed and had a very peaceful night. Why? The pain subsided to the point where I thought, "Why?" I realized that if I apply it all over, it will kill them all, otherwise the rest activate in the night.

どこで水虫に感染したのかはかなりはっきりしている。2024年の8月に狭山の病院に入院したときに感染した。つまり足掛け2年近く悩まされているのだが、それは置いておいて、入院時になぜ感染したかは、2週間に渡り入浴しなかったことと大いに関係があると思う。シャワーはあったのだが、足をよく洗った覚えはない。それに毎日シャワーを浴びた記憶もない。２,３日置きだった気がする。こうした入浴環境と入れ替わり立ち代わり患者が入れ替わるベッドの環境が、水虫にとっては最高だったのだろう。
 

It is fairly clear where the athlete's foot infection came from: I contracted it when I was hospitalized in Sayama in August 2024. That is to say, I have been suffering from it for almost two years, but putting that aside, I think the reason why I got infected when I was hospitalized had a lot to do with the fact that I did not take a bath for two weeks. I had showers, but I neither remember washing my feet very often, nor taking a shower every day. I think it was every couple of days. This kind of bathing environment and the environment of the beds, where the patients were replaced one by one, must have been the best for ringworm.

趾はだいぶきれいになってきた。ただ、まだ水虫の菌がそのあたりに巣食っているらしく、ピリピリ痛むことがよくある。そんなときにカロナールを呑むよりもっと速く効くのがインドメタシン入りの筋肉痛に効く液剤だということを最近発見した。カロナールはすぐには効かないが、皮経のインドメタシンはすぐに効く。結局、オロナイン＋水虫薬＋インドメタシン、もう少々頑張る。
 

The toes have cleared up a lot. However, there still seems to be some athlete's foot fungus nestled in the area, and I often get a tingling pain. But I recently came to know that a liquid solution for muscle pain containing indomethacin works faster than Karonal. Karonal doesn't work right away, but indomethacin skin transdermal works right away. In the end, Oronine + athlete's foot medicine + indomethacin, and a little more effort.

右脚は動脈の分岐点から鼠径部にかけて、また、その先の大腿部も血管がよく通っていた。ただ、膝からすぐ下に詰まりがあり、そのために足首から下が生っ白い色の白鬼になっていた。ASOである。反対の左足は赤鬼と呼んでいた。これは炎症があったのだろう、ずっと赤かった。それが最近は普通の色に近づいてきていた。しかし、内部の血管はとぎれとぎれの区間が長い。左もやはりASO。詰まって途切れている箇所がどのへんにあるか位置は覚えてきたから、今後はそのあたりを皮膚の上からだが揉んだり押したりすることになる。

The right leg was well vascularized from the junction of the artery to the groin and beyond to the thigh. However, there was a blockage just below the knee, which caused a raw white-colored white devil from the ankle down: ASO. The opposite left leg I called the red devil. This one was red all the time, probably due to inflammation. It had recently begun to approach a normal color. However, the internal blood vessels had a long section with a blockage. I have memorized the locations of the clogged and disconnected part of the left ASO, so from now on I will rub and press the area from above the skin.

結果は最悪、へその下辺りに分岐点があるらしいが、見せられた画像によると、分岐点のすぐ下の左側から詰まりが発生、結構その長さが長かった。またもっと下がった大腿部にはもっと長い詰まりがあるそうだ。詰まりがあるのに冷たく感じないのは、左肩のあたりからずっと血管が１本左脚まで伸びているおかげなのかもしれない。

The result was terrible. The bifurcation was said to be located below the navel, but according to the image shown, the blockage started on the left side just below the bifurcation, and was quite long. There is also a longer blockage further down on the thigh. The reason why I do not feel cold in spite of the blockage may be because there is one blood vessel extending from my left shoulder all the way down to my left leg.


手術するなら、難しい、全身麻酔のバイパス手術になるようだ。
If surgery is to be performed, it appears to be a difficult, general anesthesia bypass surgery.


「あまり手術はしたくありません、足の痛みが取れればもっと歩けるようになるから、歩いて血管の再生を促したいのですが」

'I don't want to do too much surgery, because once the pain in my leg is gone, I'll be able to walk more, and I'd like to walk and encourage the regeneration of blood vessels.'


「もちろんです。手術するかどうかはご本人の考え次第ですから」

女房はこちら抜きの医師とのやり取りの中で、手術に対してポジティブな合意をこしらえたようだった。
My wife who accompanied me seemed to have developed a positive consensus about the surgery in her communication with the doctor without me.


「もう少し猶予をいただきたいのですが」
"I'd like a little more grace."


抵抗は通った。
Resistance came through.

しかし、６月にまた検査。はたして80年使った血管がきれいになるだろうか。
However, I will have another examination in June. I wonder if the blood vessels that I have used for 80 years will get clean by then.

足と脚はよくなってきた。壊疽はないし、潰瘍も口をふさいできた。趾はさきっちょから白い部分があるゆびが多いけど、これって白癬菌の死骸じゃない?

My left foot and leg are getting better. There is no gangrene and the ulcers have closed up. Many of my toes have a white area from the tip of the toe, but aren't these dead ringworm?


ステント埋め込み手術なんかしないよ。お金もないし、入院なんかできない。歩いてないけど明日から歩く。

I'm not going to have a stent implanted. I don't have the money and I can't stay in the hospital. I'm not walking, but I'll start walking tomorrow.


膝が赤かったけど、血行不良らしいからこすった。いい感じだった。
My knee was red, but I rubbed it because apparently I have poor circulation. It felt good.


ゆびに包帯はしない。マキロンを噴射して、あとはヨードの軟膏、それにみずむし軟膏も。
No bandages on the toes. A spray of Makiron, then iodine ointment, and also a ringworm ointment.


そしてアルコールの除菌ウェットティッシュで指先を包み、それをラップで包む。
Then wrap the toes in an alcohol sanitizing wet wipe and wrap it in plastic wrap.

寒い冬の間、部屋の中に閉じこもり、ほとんど動かず、外を歩くことも大変少なかった。つけがまわってきて、何かしないと死んでしまうよというような差し迫った状態になった。そこで始めたのが椅子からの立ち上がりと座り込みの簡易スクワット10回、それに片足立ちできるだけだった。ほんとに最低限のリハビリだが、やらないよりはまし。左足での片足立ちの進歩ぶりは将来有望に思えなくもない。

During the cold winter months, I was confined to my room, hardly moving, and walking outside was very rare. The bill came due, and I felt it was imminent that I would not survive if I didn't do something. So I began to do 10 simple squats, which consisted of getting up from a chair and sitting down, and standing on one leg. It was minimal rehabilitation, but it was better than doing nothing. The progress I have made in standing on one leg with my left leg is not discouraging.

ASOは今ではPADというようだ。PAD=Peripheral Artery Disease(末梢動脈疾患)。末梢とは心臓と頭以外のことで、腹部の大動脈は末梢になる。この病気は増えつつある。主には動脈硬化によるのだが、これは若年から発生する。運動選手でもない一般人はみな、対策が必要だ。

ASO is now called PAD, where PAD = Peripheral Artery Disease. Peripheral means outside of the heart and head, and so the abdominal aorta is peripheral. This disease is on the increase. It is mainly caused by atherosclerosis, which starts at a young age. All non-athletic people need to take precautions.
 

しかし、ウォーキングもきらい、テニスなんかもっと嫌いという場合、床の拭き掃除のほかに役立つのはバイブレーターかもしれない。これはASOについて調べるうちに行き当たったのだが、金沢大学医学部（訂正）医薬保健研究域保健学系の方々がバイブレーターを用いた知見を報告しており、有意な結果が得られたとしている。
But if you don't like walking, or even worse, tennis, then a vibrator may help, in addition to mopping the floor. I came across this while researching ASO, and researchers of the Faculty of Health Sciences, Institute of Medical, Pharmaceutical and Health Sciences, Kanazawa University have reported findings using vibrators and found significant results.

世の中で表立った治療とされているのはステント留置とバイパス手術、それに運動と血液サラサラの薬だが、裏というか、目立たないエビデンスとしてバイブレーターの使用がある。このバイブレーターはきちんとした医療器具だろう。ただ、似たものとして大人のおもちゃもある。その健康のための意義をないがしろにしてはいけないということで、病院で説明を受ける前の悪あがきとして、歩けないためにバイブを多用している。ものはかつて女房が私にくれたもの。どこかからもらったものらしい。

The most popular treatments in the world are stenting, bypass surgery,  exercise, and blood-thinning medications, but there is behind-the-scenes, less obvious evidence of the use of vibrators. This vibrator should be a proper medical device. But there are adult toys that are similar, whose significance to health should not be neglected. And so, as a bad joke before the explanation given at the hospital, I use the vibrator a lot because I can't walk. The thing was once given to me by my wife. She got it from somewhere. 

我が足の惨状は、親指が白い、第2趾と第5趾も白い。白癬菌が生きて群がっているのか、死骸が溜まっている。その部分が堅いので壊疽みたいだが、石炭みたいな黒色ではないし、悪臭もしないし、全身状態も悪くない。趾のあたりに洗濯板が出現したのは懐かしい。足白癬は1度かかると厄介極まりない。

The devastation on my feet is that my thumb is white, as are my second and fifth toes. The ringworm fungus is alive and swarming, or it's a collection of dead bodies. The area is hard, so it looks like gangrene, but it is not black like coal, it does not stink, and the general body condition is not bad. I find the appearance of the washboard around my toes. Tinea pedis is extremely troublesome once you get it.

今日は病院でASOの検査。両脚の動脈の状態を超音波のエコーで調べた。スーパー音波を当てて、その跳ね返り具合から動画を作り出す。付け根から足先まで2脚、1時間あまりかかったので疲れた。その後、脚の血管のCTスキャンもやった。これは造影剤を静注してよく映るようにしてするX線撮影。短時間で済むが動いてはいけないというちょっとした拷問付き。この拷問はMRIよりは楽。

Today I was at the hospital for an ASO exam. The arteries in both legs were checked with ultrasound echoes. The super sonic waves are applied and a video is produced from the bounce back. It took about an hour for two legs from the base to the toes, so I was awfully tired. After that, I also got a CT scan of the blood vessels in the legs. This is an x-ray scan in which a contrast agent is injected intravenously to make the image look better. It takes only a short time, but it comes with a little torture: you are not allowed to move. This torture is less severe than the MRI.


果たして1週間後に結果がどう出るか。全然期待できない。ステント留置2本もありえる。

We will see what the result will be in a week. I don't expect much at all. Two stents could be implanted.


つま先のキズは、第2趾は朝血が出たから潰瘍、第5趾は歩いて非常に痛かったからまだ死んでない、生きている。

The toes with scratches are still alive, the second toe would be an ulcer because it bled in the morning and the fifth toe is not dead yet because it was very painful to walk on.

紹介された病院の若い皮膚科の医師は、こちらの趾の傷跡を見てすぐさま「壊疽(えそ)になっている」と言った。
The young dermatologist at the hospital to which I was referred looked at the scar on my toe and immediately said, "It's a gangrene."


「えーっ、えそー？うそー」
"Oh, yeah? gangrene? No way!" I thought.
 

と思ったが、 実際そうなのかもしれなかったから受け入れた。
And admitted what he said because it might possibly so.

 
2,3日後、その先生が紹介状を書いてくれた血管外科のある病院の先生は、「えそ」とは一言も言わず「かいよう」であると2回言った。
After a few days, I went to see a doctor at the hospital where vascular surgery is available. He said twice that it was "ulcer" without saying any word about "gangrene".
 

たしかに小指の外側の靴擦れのあとは色が黒くなっている。黒はえその色なのだが、紹介元の先生が言っていたそこまで黒くはない程度の黒さで、しかも、入浴後に色が薄くなったり触れると痛かったりするので、死んではいないようなのだ。
It is true that the color is black after the shoe scrape on the outside of the little toe. Black is the characteristic color of gangrene, but it is not as black as the doctor who referred me said it was, and besides, it does not seem to be dead because the color fades after bathing and it is painful to touch. It doesn't seem to be dead. 


2つ目の病院では傷の手当もしてくれた。ヨードチンキみたいだった。1つ目の病院では傷の手当はしてもらえなかった。 差が出た。
The second hospital also treated my wounds. It was like tincture of iodine; the first hospital did not treat my wounds. There was a difference.

 

今後も検査や手術が続く見込み。
Further tests and surgeries are expected to follow.

昨日のクリニックでの診察の結果、別の病院の皮膚科にいくことになった。こちらの歩行困難を見咎めた看護師さんが足を見て「水虫だけじゃない、色が悪い、すぐに皮膚科に行ったほうがいい」として先生に報告、先生がたっぷり時間をかけて照会状を書いた。右足の下肢動脈硬化が左に移った。

As a result of yesterday's examination at the clinic, we decided my going to see a dermatologist at another hospital. The nurse, who blamed my difficulty in walking, looked at my foot and reported it to the doctor as "not only athlete's foot, but also bad color, he should go to a dermatologist immediately", and the doctor took plenty of time to write a letter of inquiry. The lower extremity arteriosclerosis in the right leg came to the left.
